Health Office
Students who come to the Health Office will have their visit documented in the Health Max computerized charting system, and a written notification will be sent home with students. If there is ahead, facial, or eye injury, we will make effort to contact you in person. Attempts will be made to notify parents of any serious injury or illness. Paramedics and parents will be called in the event of a major or life-threatening injury/illness. It is vitally important that the Health Office has current phone numbers and contact information. Please, make sure to notify us immediately with any changes.
SCREENINGS
Kindergarten, 3rd grade and 5th grade students are screened for Vision and Hearing. If your child fails a screening, a written referral will be sent home with your child.
MEDICATIONS
- Any student who takes any type of medication at school must have a written parent authorization on file in the Health Office. "Medication" includes any Prescription, Asthma or Over -the-Counter (OTC) medicine, including cough drops, topical medications and eye or ear drops.
- An "Authorization to Administer Medication" form signed by the physician/provider and the parent/guardian is required for all prescription medications given for more than 14 days. Prescription medications must be in a pharmacy labeled container and must match the written physician instructions on the orders. Ask your pharmacist for a second bottle with a label on it for school use.
NOTE: the pharmacy label does NOT serve as the provider's authorization.
- An "Over-the-Counter (OTC) or Short-Term Prescription" form must be signed by the parent/guardian for all OTC non-prescription or short-term (1 - 14 days) prescription medications/remedies.
- Medications must be in the original containers (no medicine in baggies).
- OTC/non-prescription medications will not be given for more than 5 consecutive days, unless the "Authorization to Administer Medication" is provided.
- Asthma medications must have a physician's written authorization and a parent signature to the "Asthma Medication Authorization" form. If your child self-carries an inhaler, it is strongly advised to have a back-up inhaler in the Health Office. Student should be able to self-administer their medication with supervision. Each child should know the name of their medication, the dose and why they are taking this medication.
- All medications authorizations are valid for the current school year and need to be renewed before the start of the next school year.
- Medications (with the exception of asthma inhalers) will not be sent home with a student. A parent/guardian or other designated adult must pick-up a medication from the Health Office.
